After my mom had her stroke and some follow-up pneumonia it was recommended to me by the hospital to put my mom into hospice. Together Hospice has been a great partner. Joseph is extremely responsive and helps in any way that he can as situations arise. Joseph also helped me find a good board and care facility that I am very happy with. It's such a tough thing to know who to trust during these times, and Joseph has provided a lot of resources throughout this experience.

Nurses are accessible to speak with and do their best to turn around quick resolutions.

I would recommend Together Hospice to someone going through these tough times.

Words cannot express how thankful I am for Together Hospice. It can be a daunting task when trying to find the proper care and facilities for our loved ones. Joseph went above and beyond to make this process as quick and stress-free as one could imagine. Where other social workers from other facilities could not find solutions for weeks on end, Joseph from Together Hospice got results almost immediately. He also just as quickly connected us with financial aid . On top of that, the facilities that he works with are all vetted and as he would put it: "up to the standards he would want if it were his loved one." And this is because it was once his loved one. He has been where we are, and he conducts himself with a passion and empathy that reflects this. The result: my father now has a place to stay in walking distance from me, with excellent staff and facilities. This all took place within a matter of days. I could not have asked for nor gotten better results much less anywhere near to the same results with anybody else. If you want the best, look no further.

Joseph, Patrick, Cynthia and the rest of the team at Together Hospice have gone above and beyond in assisting with the care of my grandmother. They helped us find a caregiver that my grandmother is comfortable with and constantly followed up with our family to make sure we have everything we needed in order for my grandmother to be able to stay at home during her transition.
